We have started an aftercare support service in which we provide support, advice and communication to students after they graduate to help them maintain the positive behavioral progress that they have made during their time at JRC. The aftercare services are coordinated by the student’s clinician, who sets up an aftercare plan with the student and his/her outside treatment team and family prior to the student leaving JRC. This collaborative approach attempts to integrate multiple facets of a students support network after leaving JRC, and provides for a smooth transition back into a student’s community.

The clinician sets up a series of regularly scheduled phone calls with the student to discuss his/her progress, possible obstacles he/she may have encountered, and steps that the student might take to correct any setbacks. The clinician may also continue to have regular conversation with the student’s parent or guardian to discuss their child’s transition back to the home, potential difficulties that they may be having, and strategies that may be helpful in optimizing the new living situation. Finally, the clinician may have contact with the student’s outside treatment team to ensure continuity of care.

In certain cases, students have requested and received assistance to establish behavioral self-management projects to help them accomplish behavioral goals such as losing weight.

Former students are invited to participate in the Student Discussion Board, to attend our annual Graduation/Culmination ceremony, and to attend, at JRC’s expense, an annual reunion weekend following Graduation/Culmination day.

Additional services may also include bringing a student back to JRC for a visit, providing training to an outside agency, and participation in an ongoing follow-up study. This follow-up study is an ongoing project, year after year. In the course of collecting information, members of our staff communicate with each former student at least once every year.
